Job summary

Hospice of the West is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our nursing team serving Casa Grande and surrounding community. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the journey with compassion and clinical excellence.

The role requires an active RN license, at least 1 year in medical-surgical/acute care, EMR proficiency, CPR (if required), and reliable transportation.

Qualifications

• Must hold active RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
• Proficient with EMR systems.
• CPR certification as required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Empower through engagement by responding to referral information with care and expertise.
• Collaborative care champion by partnering with Clinical Director to assess eligibility for hospice services.
• Nurturing direct care as an RN Case Manager, ensuring comfort and quality of life.
• Nursing with heart by delivering care rooted in proven principles and techniques.
• Educator and advocate, educating patients and families to create personalized care plans.
• Share call duties as part of the team to be there when it matters most.
