JourneyCare Hospice is seeking a Hospice RN to join our team in Highland Park, Lake Forest, and Northbrook, IL. The role involves managing patient and family caseload, coordinating with the interdisciplinary team, and delivering comprehensive hospice care.

Qualifications

• Graduated from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation.
• Obtain data from patient physical, psychological, social, and spiritual factors to impact care and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
