JourneyCare Hospice is seeking a dedicated Hospice RN to join our team in Highland Park, IL. The role involves managing patient and family caseloads, coordinating care, and ensuring high-quality nursing services with a compassionate approach.

We offer a salary range of $80,000–$97,000 annually, with a comprehensive benefits package including 401(k) with match, generous time off, and tuition reimbursement. Hours are Monday–Friday with weekend rotation and holiday coverage.

Competenze

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance are required.

Mansioni

• Assume primary responsibility for patient and family caseload, including assessment, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care.
• Provide regular visits to re-assess the patient's needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines.

HOURS

Monday-Friday 8:30am-5pm Plus One Weekend Every 12th Week (Saturday and Sunday) and One Summer and One Winter Holiday
