Job summary

Choice Health at Home is seeking a Hospice RN Case Manager to provide patient-centered nursing care in home and care settings in Colorado Springs, CO. The role emphasizes coordinating comprehensive care plans, collaborating with interdisciplinary teams, and ensuring comfort and dignity for patients and families.

The Hospice Field RN works independently with strong clinical judgment, excellent communication, and the ability to adapt to dynamic environments.

Qualifications

• Current RN license is required.
• CPR certification is required.
• Valid driver’s license and auto insurance are required.
• At least 2 years Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients and document findings.
• Apply admission and re-certification criteria for hospice care to determine level of care.
• Develop, implement, and update the plan of care to ensure quality and continuity of care.
• Educate patient and family on disease process, self-care, and end-of-life preferences; train staff as needed.
• Determine service scope and frequency based on acuity and family needs; assess caregiver readiness.
• Provide clinical directions to Hospice Aide and LPN/LVN to maintain care quality.
• Prepare progress notes documenting progress toward goals and ensure proper documentation.
• Inform physicians of changes in patient needs and response to care; monitor outcomes.
• Perform on-call duties and provide on-call coverage as assigned.
