Job summary

Moments Hospice is seeking an RN Case Manager to deliver high-quality care and coordinate patient plans in a road-based role. You will assess patients, guide care plans, and collaborate with families and the care team to ensure comfort at the end of life.

The position includes a company car with fuel and insurance covered, extensive health benefits, generous PTO, and a 401(k). A valid RN license and EMR proficiency are required.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning
