Job summary
Moments Hospice is seeking an RN Case Manager to deliver compassionate, high-quality in-home and on-the-road care. You will assess patients, coordinate plans, educate families, and collaborate with an interdisciplinary team while traveling to patient homes.
Eligible candidates are licensed RNs, proficient with EMR, with strong communication and autonomous problem-solving abilities. A valid driver's license and reliable vehicle with insurance are required, and you will join a supportive

- Active RN license in state of work.
- Proficient with Electronic Medical Records (EMR) software.
- Excellent planning, organizing, and communication skills.
- Ability to solve problems autonomously while meeting high standards.
- Valid Driver's License and reliable vehicle with current insurance.
Responsibilities
- Conduct initial client assessments and continuously refine care plans.
- Provide hands-on nursing care and patient education.
- Maintain open lines of communication with patients, families, and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
