Job summary

VITAS Healthcare is seeking a Home Health RN to provide compassionate end-of-life care in patients' homes, including private residences and facilities. You will be part of a respected team delivering comfort and dignity at the end of life.

You will collaborate with an interdisciplinary team, visiting 4–6 patients daily, educating families on palliative care, and ensuring patient welfare and comfort remains the top priority.

Qualifications

• RN license in the state where the program is located.
• Minimum two years nursing experience, preferably in hospice/home health.
• Reliable transportation to visit patients’ homes.
• Experience delivering compassionate, patient-centered care.

Responsibilities

• Provide end-of-life nursing care in patients’ homes.
• Visit 4–6 patients daily.
• Collaborate with interdisciplinary team weekly.
• Educate patients and families on palliative care.
• Maintain patient welfare and dignity at all times.
