Job summary

Qualifications

• Currently licensed to practice nursing in the state where the program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health within the last five years.
• Equivalent experience or licensure may be considered.

• Provide end-of-life nursing care in patients' homes and document care plans.
• Collaborate with an interdisciplinary team to support patients and families.
