Job summary

VITAS Healthcare seeks a compassionate Home Health Registered Nurse (RN) to deliver end-of-life care in patients’ homes. You will build meaningful connections with patients and families while guiding them through the palliative approach and comfort-focused care.

Daily duties include direct patient care, collaboration with an interdisciplinary team, and education on hospice principles. This role is field-based, with visits across assigned service areas and a commitment to dignified end-of-life

Qualifications

• Currently licensed to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered

Responsibilities

• Provide end-of-life care and comfort-focused nursing to hospice patients in their homes.
• Coordinate with an interdisciplinary team to support patient and family needs.
• Educate patients and families about palliative care and the hospice approach.
• Make 4–6 patient visits per day and document care appropriately.
