Job summary

VITAS Healthcare is seeking a Home Health RN to ensure hospice patients are comfortable in their homes or facilities and to form compassionate connections with families. You will work within an interdisciplinary team to guide care decisions at the bedside.

You will typically make 4–6 home visits daily, attend weekly team meetings, and uphold the company’s commitment to extending comfort and dignity at end-of-life. Valid RN license and reliable transportation are required.

Qualifications

• Must have an active RN license.
• Minimum of two years of nursing experience.
• Reliable transportation to visit patients at home or facilities.

Responsibilities

• Provide hospice care and comfort to patients in home or facility settings.
• Build compassionate relationships with patients and families.
• Collaborate with an interdisciplinary team and document care.
• Visit patients 4–6 times per day and coordinate with the program office weekly.
