Job summary

Northern Arizona Hospice, part of LHC Group, is hiring an RN Case Manager to oversee the Plan of Care for hospice patients in a full-time role. You will evaluate care needs, coordinate with the care team, and implement treatment strategies that support physical, psychosocial, and spiritual well-being.

Requirements include active RN licensure in Arizona, CPR certification, a valid driver's license, and 1+ year of clinical experience. Benefits are offered.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ year of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Initial nursing evaluation for eligibility and hospice care within 48 hours of referral.
• Identify patient/family needs and re-assess every 15 days.
• Initiate and coordinate the plan of care with the IDG and attending physician.
• Document problems, goals, interventions, and patient/family responses.
• Educate and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes.
• Provide care regardless of patient background.
