UnitedHealth Group is looking for a Registered Nurse Case Manager in Mountain Home, Arkansas. You will take full nursing responsibility for each hospice patient's care plan, performing evaluations and assessments, and coordinating care with healthcare professionals. Candidates must hold a current RN license and preferably have over a year of clinical experience. Qualifications
Current and unrestricted RN licensure in the state of practice.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
1+ years of clinical experience preferred.
Current CPR certification or ability to complete within 90 days of hire.
Responsibilities
Conduct initial nursing evaluations within 48 hours of referral.
Identify and reassess patient/family needs at least every 15 days.
Initiate and coordinate the plan of care.
Document treatment goals and patient responses.
Collaborate daily with patients, families, and healthcare team.
Instruct and supervise self-care techniques for patients.
Maintain accurate clinical notes on patient conditions.
