Job summary

Baptist Hospice, part of LHC Group and within the Optum family, is seeking an RN Case Manager to deliver and evaluate Plan of Care for hospice patients in the Mississippi area. You will work with a multidisciplinary team to promote physical, psychosocial, emotional, and spiritual well-being, including care for patients facing end-of-life concerns.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• MS: 1+ years of full-time RN experience; three years as an LPN may substitute for the RN requirement.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Provides care in diverse home situations regardless of age, race, creed, or disability.
