Job summary

Louisiana Hospice and Palliative Care, part of the LHC Group within the Optum family, seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will collaborate with professionals to implement treatment strategies and promote holistic well-being for patients and families.

Responsibilities include evaluating eligibility, coordinating care, documenting progress, and guiding families in self-care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to dependable vehicle or public transportation.
• State-specific requirement: 2+ years of full-time RN experience in hospice care in LA; substitution allowed with 2 years as an RN replaced by 2 years as a licensed practical nurse.

Responsibilities

• Make initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician, and IDG members in daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
