Job summary

Optum is seeking a Hospice Registered Nurse (RN) to manage the Plan of Care for hospice patients, performing evaluations, coordinating care with the IDG, and supporting families at home. The role emphasizes quality of care, clinical documentation, and collaboration with physicians.

The ideal candidate holds an active RN license in Mississippi, has reliable transportation, and is prepared for a role requiring compassion in end-of-life care. CPR certification is preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• MS: 1+ years of RN experience; substitution with 3 years LPN allowed.

Responsibilities

• Perform initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient's/family's physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions and responses.
• Collaborate with patient/family, attending physician and IDG to provide care daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
