Baptist Hospice, part of the Optum family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in Mississippi. You will evaluate performance, collaborate with professionals, and adjust treatment plans to support physical, psychosocial, emotional, and spiritual well-being.

The role emphasizes thorough nursing evaluations, family education, and coordination with the IDG while maintaining compassionate care in diverse home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• MS state-specific requirement: 1+ years of full-time RN experience; LPN substitution permitted.
• Preferred: CPR certification or ability to obtain within 90 days of hire.

Responsabilités

• Perform initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Assess physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the patient’s plan of care.
• Document problems, goals, interventions, and patient/family responses.
