Job summary

UnitedHealth Group's CHRISTUS Hospice and Palliative Care SPOHN, part of LHC Group, seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will evaluate performance and collaborate with professionals to promote well-being, including patients and families facing end-of-life situations.

Responsibilities include initial hospice eligibility evaluation within 48 hours, coordinating and documenting care, collaborating with the IDG, and teaching families self-care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate with patient/family, attending physician and IDG members for daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition and progress.
