Job summary

Optum is hiring a Registered Nurse Case Manager in Louisiana to deliver hospice services under the Plan of Care and collaborate with a multidisciplinary team to promote physical, psychosocial, and spiritual well-being for patients and families.

You will evaluate performance, develop treatment strategies, and manage care plans in diverse home settings, with a focus on compassionate, patient-centered care and adherence to clinical guidelines.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies patient/family needs and re-assesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care and documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician and IDG members to provide care daily.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
