Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for CHRISTUS Hospice St. Frances Cabrini in Louisiana. The nurse will oversee Plan of Care delivery for hospice patients, continually evaluate performance, and ensure quality patient care in alignment with hospice policies.

This role emphasizes collaboration with families, physicians, and the care team. Requirements include an active RN license in Louisiana, 1+ year clinical experience, CPR certification, and a valid driver's license

Qualifications

  • Current unrestricted RN licensure in the state of practice
  • 1+ years of clinical experience
  • Current CPR Certification
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
  • LA state requirement: two years RN experience or two years hospice experience substituted for RN

Responsibilities

  • Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral
  • Identifies patient/family needs and re-assesses at least every 15 days
  • Initiates and coordinates the plan of care with accurate clinical notes
  • Documents problems, goals, interventions, and patient/family responses
  • Collaborates with patient/family, attending physician, and IDG for daily care
