Job summary

Angels Care Hospice in Texas seeks a Registered Nurse Case Manager to coordinate all aspects of home care for patients in their homes. You will interact with medical teams and provide professional patient care, including admission visits to assess eligibility and to develop the care plan.

The role requires RN licensure, CPR, a valid driver's license, and reliable transportation, with 1 year of RN experience preferred. We offer paid time off, benefits, and opportunities for growth.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The RN Case Manager coordinates all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visits to determine eligibility for home care services.
• Develop and follow the patient’s plan of care.
