Job summary

Luminary Hospice seeks a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. Requires strong nursing experience with emphasis on community health education and coordination with the interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing (required).
• Current licensure in State and CPR certification.
• Bachelor’s degree; hospice experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive patient/family assessment to identify hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient/family and provide health instruction.
• Coordinate patient care as Case Manager with clinical team.
