Job summary

Affinity Health Management is seeking a Clinical Manager to coordinate patient care within hospice and home care services. The role supervises clinical staff and ensures services are delivered appropriately, with continuous attention to quality and compliance.

The RN will review cases, manage medical records in EMR, and participate in case conferences to promote effective interdisciplinary collaboration and patient-centered care.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN preferred; or minimum 1 year of nursing experience within the last 3 years.

Responsibilities

• Receives case referrals and reviews patient information to determine hospice/home care needs.
• Assigns hospice personnel to cases and consults Medical Director on eligibility questions.
• Reviews and evaluates cases to guide clinicians and improve home care services.
• Manages medical records and workflow in EMR system.
• Assists in establishing goals and plan of care with clinicians.
• Attends case conferences to coordinate care among hospice staff.
• Performs quarterly record reviews and communicates findings to Clinical Director and staff.
• Assists in interviewing and orientation of new employees.
• Contributes to education program planning and policy development.
• Ensures compliance with professional standards and hospice philosophy.
• Participates in community health planning and quality improvement activities.
• Performs other duties as delegated by the Clinical Director.
