Qualifications

• RN with active license in the state.
• BSN preferred or 1 year nursing experience.
• Hospice/home care experience and 2 years management.
• Maintains CPR certification.
• Complies with professional standards.
• Has driving license and insured vehicle.
• Understands hospice philosophy.
• Excellent observation and communication skills.

Responsibilities

• Receives case referrals and reviews patient information to determine hospice/home care needs.
• Assigns appropriate hospice personnel to cases and consults with Medical Director on eligibility.
• Reviews and evaluates cases through home visits, conferences, records review, and clinician services; guides clinicians.
• Manages medical records and EMR workflow.
• Assists clinicians in setting immediate and long-term goals and care plans.
• Attends case conferences to coordinate care and interdisciplinary involvement.
• Performs quarterly record reviews and communicates findings to leadership.
• Assists in screening and interviewing new employees and orientation.
• Contributes to development of policies and organization goals.
• Ensures compliance with professional standards and practices.
• Assesses community health needs and participates in program development.
• Participates in quality improvement teams.
• Performs other duties as delegated by Clinical Director.
