Job summary

Affinity Hospice is a rapidly growing national hospice provider seeking a Clinical Manager to oversee clinical services and coordinate patient care. You will direct interdisciplinary teams, guide clinicians, and ensure high-quality outcomes for our patients and families.

The role requires an RN with state licensure, leadership experience in hospice/home care, CPR certification, and a valid driver’s license. BSN is preferred as we expand our skilled nursing leadership.

Qualifications

• Registered Nurse with current licensure to practice professional nursing in the state.
• Two years of management or supervisory experience in a hospice or home care setting.
• BSN preferred; NLN-accredited nursing program.
• Valid driver’s license with insured automobile.
• Current CPR certification.

Responsibilities

• Review case referrals and assess patient needs, including home visits.
• Assign hospice associates and collaborate with Medical Director to determine eligibility for services.
• Oversee and evaluate patient cases through home visits, conferences, and record reviews.
• Provide guidance to clinicians to improve performance and service delivery.
• Lead case conferences with hospice associates to ensure care coordination.
• Participate in hiring, screening, and orientation for new associates, providing training and mentorship.
• Assist in planning, implementing, and evaluating in-service and continuing education for staff.
