Qualifications

• Registered nurse with current state licensure.
• BSN preferred or minimum 1 year nursing experience within last 3 years.
• Two years of management or supervisory experience in hospice/home care.
• Complies with professional standards and CPR certification current.
• HCHB and PointCare experience preferred.

Responsibilities

• Coordinate patient care and supervise clinical personnel.
• Review cases through home visits, conferences and chart reviews to guide care delivery.
• Maintain and review medical records in the EMR system.
• Assist in establishing immediate and long-term therapeutic goals and plan of care.
• Participate in case conferences and quality improvement activities.
• Assist in hiring, orientation and ongoing staff development.
