Job summary

Avow is seeking a Registered Nurse Case Manager in Florida to identify patient and family needs and coordinate comprehensive care under physician orders. The role emphasizes documenting assessments in the EMR, updating the interdisciplinary plan of care, and guiding families through care and medication regimens.

The ideal candidate will hold a Florida RN license, possess strong computer skills, and communicate effectively in English (Spanish a plus).

Qualifications

• Bachelor’s degree (or equivalent) from four-year college/university or 1–2 years related experience.
• Current RN License to practice in Florida.
• Strong computer skills including database software and Outlook.
• Spanish language skills preferred.

Responsibilities

• Assess and identify patient/family needs under physician orders to plan care.
• Document assessments in EMR in a timely manner.
• Coordinate care with interdisciplinary team and outside services.
• Develop and update the plan of care with input from the team.
• Teach patient/family about care plans and medications as needed.
• Ensure staff and caregivers understand and follow the care plan.
