Job summary

Agape’s Way Healthcare Services is seeking an experienced RN Case Manager to provide clinical support across the patient journey in Florida. You will perform in-person pre-admission bedside assessments, coordinate admissions, and manage clinical records and physician orders within a multidisciplinary team.

The role requires strong clinical judgment, care coordination, and documentation skills, with collaboration across nursing, physicians, referral partners, and billing to ensure high-quality

Qualifications

• Current Florida RN license.
• Graduate of an approved nursing education program.
• BSN preferred.
• Minimum of two years of professional nursing experience.
• Home health, case management, or care coordination experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct pre-admission bedside assessments and coordinate initial nursing records.
• Coordinate physician and allowed-practitioner appointments and required documentation.
• Maintain oversight of caseload and ensure timely follow-ups.
• Identify missing or time-sensitive documentation and escalate when needed.
• Collaborate across departments to align services with the care plan.
• Support admissions, transfers, and discharges while ensuring continuity of care.
