Job summary

Empath Health is seeking a PRN RN Case Manager to join our hospice team in southwest Florida. You will coordinate and update care plans, educate patients and families, and deliver compassionate nursing care across home, LTC facilities, or assisted-living settings.

The role requires Florida RN licensure, an approved nursing program graduate, and ideally BSN/MSN with at least one year of acute care or hospice experience.

Qualifications

• Bedside hospital, ICU, ER or other inpatient experience within the past 10 years.
• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure: Valid Florida license as a Registered Nurse
• Minimum of 1 year of current active nursing experience; hospice experience preferred.

Responsibilities

• The PRN RN Case Manager creates and maintains a plan of care, coordinating patient and family interaction with Tidewell Hospice.
• Reviews and updates the care plan, physician orders, and pharmacy profile; educates patients and families about the disease process and palliative options.
• Serves patients and families in home, long-term care facilities, or assisted-living facilities.
• Provides nursing care in emotionally intense situations with listening, observation, and clear communication.
