Job summary

Avow Hospice is seeking an RN Case Manager to identify patient and family needs and coordinate care to maximize comfort and quality of life in the Naples area. The role involves visiting patients in Collier County, following physician orders, and coordinating with the interdisciplinary team to implement and adjust care plans.

The position requires a Florida RN license, a Bachelor’s degree or equivalent experience, and strong computer skills including EMR and Outlook.

Qualifications

• Bachelor’s degree from four year college or university or equivalent combination of education and experience.
• Excellent reading, writing, and speaking English communication skills. Spanish preferred.

Responsibilities

• Assess physical and emotional needs of patients and plan care.
• Complete Patient Assessment Form/Notes in EMR.
• Recognize and report changes in patient condition to clinical team.
• Coordinate supplies, equipment, and medications as needed.
• Develop and update the interdisciplinary plan of care.
• Educate patient/family and participate in IDG planning.
• Administer medications with knowledge of contraindications and outcomes.
• Communicate with care team and home health aides about care plans.
