Job summary

Peoples Health Services in the Florida Panhandle seeks a Registered Nurse to assess, plan, implement, and coordinate patient care. You will develop and monitor a Plan of Care based on clinical guidelines and utilize OASIS data to promote quality, cost-effective outcomes.

Excellent communication and collaboration with physicians and the clinical team are essential, with proficiency in PPS, OSHA, JCAHO/CHAP standards and payor guidelines.

Qualifications

• Current RN license in the State of Florida.
• Current CPR certification.
• Current driver’s license and valid auto insurance.
• Completion of 25 CEU’s bi-annually.

Responsibilities

• Performs client admissions, resumption of care, follow-up using OASIS data collection in a timely, legible, accurate manner.
• Responsible for monitoring and managing core measurements pertinent to the department to assure compliance with the operating plan.
• Identifies, documents and reports signs and symptoms of altered health status to physician and Clinical Team Manager.
• Develops Plan of Care based on assessment findings and guidelines; sets measurable goals and outcomes.
• Administers meds and treatment according to Plan of Treatment.
• Teaches nursing care and reflects in records.
• Coordinates care with other health team members and participates in conferences.
• Documents client records timely and accurately; completes plan of care and medication records.
• Notes progress towards discharge planning and communicates with physician and staff.
• Completes discharge summaries.
• Supervises LPN and Home Health Aide; conducts supervisory visits.
• Utilizes nursing, PPS, and case management guidelines to promote positive outcomes.
