Job summary

Bristol Hospice is seeking a Registered Nurse Case Manager who delivers compassionate, whole-person care and advocates for patients and families throughout the hospice journey. You will develop and implement individualized care plans, coordinate across teams, and ensure comfort, dignity, and understanding at every step.

The role requires a valid RN license, CPR certification, a valid driver's license, and strong written and verbal communication.

Qualifications

• Active RN license in good standing in the state you will work in.
• CPR certification (current) is required.
• Valid driver’s license and reliable transportation.
• Strong observational and communication skills—written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs accurately (temperature, pulse, respiration, blood pressure).
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely responses.
• Maintain detailed clinical notes reflecting patient progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
