Bristol Hospice is seeking an RN Case Manager who will provide compassionate, whole-person care and act as an advocate for patients and their families. The role emphasizes walking beside patients through every step of the hospice journey and delivering individualized, dignified support.

Qualifications

• Must hold a valid Registered Nurse license in the state you will work in.
• CPR certification is required.
• Excellent verbal and written communication skills are essential.

Responsibilities
