Job summary

UnitedHealth Group's Caretenders Hospice is seeking a Registered Nurse Case Manager to deliver and evaluate the Plan of Care for hospice patients. You will collaborate with the IDG, implement treatment strategies, and adapt plans to promote physical, psychosocial, emotional, and spiritual well-being.

Both full-time and PRN options exist. Applicants must hold a current RN license in the state, have a valid driving license or reliable transportation, and preferably CPR certification.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with the IDG and attending physician.
• Documents goals, interventions, and patient/family responses to hospice care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and provides care regardless of patient background.
