Job summary

UnitedHealth Group's Heart n Home Hospice is seeking a Registered Nurse Case Manager to lead the Plan of Care for hospice patients in home settings. You will collaborate with the IDG to implement treatment strategies that support physical, psychosocial, emotional, and spiritual well-being, adapting care as patient needs evolve.

Responsibilities include initial eligibility evaluation within 48 hours of referral, coordinating care, documenting progress, and educating patients and families in

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family needs and re-assesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care and documents goals, interventions, and responses.
