Job summary

Affinity Hospice Holdings, LLC is seeking a Registered Nurse Case Manager to plan, organize, direct and provide compassionate hospice care in patients' homes and communities.

You will lead an interdisciplinary team, develop patient care plans, and ensure documentation, compliance, and quality standards are met.

This full-time role requires a valid RN license, CPR certification, a driver’s license, and strong communication skills; a $5,000 sign-on bonus is offered.

Qualifications

• RN licensure in state of practice.
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice preferred.
• Able to drive and maintain insured vehicle as required.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document patient plan of care.
• Provide care with infection control measures protecting staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
