Job summary

Gentiva is seeking a compassionate Hospice RN/Case Manager to deliver personalized end-of-life care in patients' homes and other care settings. You will lead comprehensive assessments, develop plans of care, and coordinate with an interdisciplinary team to ensure comfort, dignity, and patient-centered support.

Responsibilities include documenting care, educating families, and supporting bereavement needs, while adhering to regulatory standards and maintaining meticulous records.

Qualifications

• RN license in state of employment.
• Current CPR certification.
• Auto insurance and valid driver’s license.
• Hospice or palliative care experience preferred.
• Ability to perform home health nursing assessments.

Responsibilities

• Act as primary contact for patients and families through the hospice journey.
• Deliver and document skilled nursing care per the Plan of Care at home or in care settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG and physicians to update personalized care plans.
• Provide education and bereavement support to patients and families.
• Educate caregivers on safe patient handling and self-care.
• Maintain accurate clinical documentation and infection control compliance.
• Support care continuity by coordinating referrals and transitions.
• Participate in on-call and respond to patient needs.
