Job summary

Allay Hospice is seeking a compassionate RN Case Manager to deliver patient-centered hospice care in the Cedarburg area. You will join a supportive team and provide skilled nursing care, pain management, and family education while coordinating with social work, chaplaincy, aides, and providers.

The role emphasizes independent work within a collaborative environment, with a strong emphasis on documentation accuracy and adherence to hospice philosophy.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with automobile insured and in good working order

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
