Job summary

Allay Hospice is seeking a dedicated Hospice RN Case Manager to provide patient-centered end-of-life care in the Wisconsin region. You will work with an interdisciplinary team to plan and deliver compassionate care, coordinating with social work, chaplaincy, aides, and physicians.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the operating state.
• Hospice experience or exposure is preferred; passion for patient-centered care.
• Ability to collaborate with an interdisciplinary team and document care precisely.

Responsibilities

• Assess and monitor physical, emotional, and clinical needs of patients.
• Create and update hospice care plans aligned with patient goals.
• Direct nursing care including medications, treatments, and symptom control.
• Educate families and caregivers; maintain timely, accurate documentation.
• Collaborate with the team and participate in on-call rotation as required.
