Job summary

Allay Hospice is hiring a Registered Nurse Case Manager to join our hospice team in Wisconsin. You will provide direct nursing care, create care plans, and coordinate interdisciplinary support to deliver compassionate, patient-focused end-of-life care.

Responsibilities include assessment, pain and symptom management, family education, thorough documentation, and on-call participation. A $5,000 sign-on bonus is offered and a strong team environment supports professional growth.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the state of operation.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and documentation excellence.
• Must have a valid driver’s license and insured automobile.

Responsibilities

• Assess and monitor patients' physical, emotional, and psychological needs.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management and comfort care.
• Educate and support family and caregivers.
• Collaborate with interdisciplinary team and document thoroughly.
• Participate in on-call rotation as required by local branch.
