Job summary

Allay Hospice in Brookfield, WI is seeking a passionate Hospice RN Case Manager to deliver patient-centered care in a supportive, interdisciplinary team environment. You will assess needs, develop care plans, administer medications, and coordinate with Social Work, Chaplaincy, Aides, and providers.

This role offers a $5,000 sign-on bonus, travel across Waukesha and Milwaukee Counties, and a Monday–Friday schedule with 8 AM–5 PM.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with a valid insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
