Job summary

Heartland Hospice is seeking an RN Case Manager to lead in-home hospice care. You will perform comprehensive assessments, deliver skilled nursing care, and develop and monitor Plan of Care tailored to each patient, coordinating with physicians, LPNs, CNAs, social workers, and chaplains.

The role emphasizes patient and family education, emotional and spiritual support, and thorough documentation while ensuring compliance and quality improvement.

Qualifications

• RN license in applicable state
• 1 year RN experience in hospice/home health preferred
• CPR certification required

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice care
• Deliver and document skilled nursing care per Plan of Care at home or other settings
• Perform initial and ongoing patient assessments and adjust care as needed
• Collaborate with an interdisciplinary team to build personalized care plans
• Provide emotional, physical, and spiritual support to patients and families
• Educate caregivers and provide practical training for safe, confident care
• Communicate updates to care team and ensure timely interventions
• Maintain accurate clinical documentation and follow infection control standards
• Support quality improvement and scheduling to ensure compassionate, efficient care
• Explain hospice concepts, medications, and end-of-life processes to patients/families
