Job summary

Jefferson Health at Home by BAYADA Hospice RN is sought to provide palliative care at home within Montgomery and Bucks counties, PA, as part of a multi-disciplinary team. You will perform home visits and deliver skilled nursing care in a compassionate, patient-centered manner.

The role supports flexible schedules (full-time, part-time, per-diem, and on-call), travel to cases, 24/7 clinical support, and ongoing education opportunities while maintaining BAYADA's high standards of care.

Qualifications

• Active RN license in PA.
• One year hospice experience preferred.
• Willingness to travel to assigned cases.
• Strong communication and assessment skills.
• Ability to deliver compassionate, reliable patient care.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments and comfort care.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up with and document doctors’ orders.
• Occasional on-call after hours and/or weekends.
