Job summary

BAYADA Home Health Care and its Jefferson Health at Home by BAYADA Hospice unit are recruiting a compassionate Hospice RN Case Manager to serve patients in Montgomery and Bucks Counties, PA, delivering in-home palliative care. The role requires making home visits, administering medications, wound care, and coordinating with a multidisciplinary team.

Travel to cases is required; previous hospice experience is preferred.

Qualifications

• Active and current RN license in PA.
• One year hospice experience preferred.
• Willingness to travel to cases within designated territories.

Responsibilities

• Make home visits to patients in designated territories.
• Administer medications, wound care and treatments; provide comfort care.
• Monitor patient conditions and report changes to the team.
• Document doctors' orders and ensure proper follow-up.
• Occasional on-call after hours and/or weekends.

What you’ll do:

As a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will:

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends

What you should have:

• Active and current license to practice as a professional registered nurse (RN) in the state of PA.
• One year of hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.
