Job summary

Jefferson Health at Home by BAYADA Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to provide quality palliative care at patients' homes across Montgomery and Bucks Counties, PA. You will perform home visits, administer medications, wound care, and coordinate with a multidisciplinary team to ensure comfort and safety.

Ideal candidates hold an active PA RN license, have hospice experience, and can travel to assignments.

Qualifications

• Active and current PA RN license.
• One year hospice experience strongly preferred.
• Willingness to travel within designated territories in PA.

Responsibilities

• Make home visits to clients in defined geographic territories.
• Administer medication, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to supervisor.
• Follow up on doctors' orders and document actions.
• Occasional on-call after hours and weekends.

We are currently recruiting for a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients throughout in small territories throughout Montgomery County and Bucks County, PA.

What you’ll do:

As a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will:

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends

What you should have:

• Active and current license to practice as a professional registered nurse (RN) in the state of PA.
• One year of hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.
