Job summary

Jefferson Health at Home by BAYADA Hospice RN is seeking a compassionate registered nurse to visit patients at home across Montgomery and Bucks Counties, PA. You will work within a multidisciplinary team to deliver palliative care and manage medications, wound care, and comfort measures.

The role involves home visits, documentation, and coordination with physicians, with potential after-hours coverage. A PA RN license and hospice experience are highly valued, and travel is required.

Qualifications

• Active PA RN license.
• Hospice experience preferred.
• Home health experience welcomed but not required.
• Ability to travel to cases as assigned.
• Strong communication and assessment skills.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Performing medication administration, wound care, treatments, and comfort care.
• Monitoring clients' conditions and reporting changes to the Clinical or Client Services Manager.
• Following up with, executing, and documenting doctors' orders.
• Occasional on call after hours and/or weekends.
