Resource 1 Homecare Staffing in Woodbury, MN is seeking a Hospice Registered Nurse (RN) Case Manager to coordinate high-quality end-of-life care. This role is 40 hours per week, based in patient homes and facilities, with on-call expectations.

The RN Case Manager will assess patients, develop and revise plans of care, educate families, and collaborate with an interdisciplinary team to deliver compassionate hospice services.

Qualifications

• A passion for hospice care and the ability to build relationships with patients and families.
• Ability to work efficiently with little direct supervision.
• Valid driver's license and insured automobile in good working order.
• 1–2 years of recent acute care in an institutional setting; 1–2 years in home care preferred.
• Graduation from an accredited school of nursing.

Responsibilities

• Completes initial, comprehensive assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient/family input.
• Coordinates patient care and participates in on-call duties.
• Educates patients and families on care, medications, and home safety.
