Qualifications

• A passion for hospice care and the ability to build relationships with patients and families.
• Ability to work efficiently with little direct supervision.
• Valid driver's license and insured automobile in good working order.
• 1-2 years of recent acute care experience in an institutional setting; 1-2 years of home care experience preferred.

Responsibilities

• Completes initial, comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and initiates a care plan in collaboration with patient, family, and interdisciplinary team.
• Coordinates patient care for assigned caseload and participates in on-call duties.

Graduation from an accredited school of nursing
