Job summary

Superior Hospice is seeking a dedicated Hospice RN Case Manager in the Greater Twin Cities area. You will manage a personal caseload, delivering compassionate, patient-focused end-of-life care in patients’ homes and facilities.

You’ll coordinate with physicians, social workers and chaplains, maintain EMR documentation, and participate in weekend/on-call rotations to ensure seamless continuity of care for families during life’s most important moments.

Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred or strong background in oncology, ICU, palliative care or home health.
• Strong clinical judgment and ability to work independently.
• Excellent communication, organization and critical thinking skills.
• Passion for relationship-centered, patient-focused care.

Responsibilities

• Manage your own hospice caseload as the primary RN for patients and families.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Provide expert pain and symptom management focused on comfort and quality of life.
• Guide and educate patients, caregivers, and families throughout the hospice journey.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists and community partners.
• Provide care in patients' homes, assisted living communities and SNFs.
• Maintain accurate, timely EMR documentation while ensuring Medicare hospice compliance.
• Participate in weekend and on-call rotation to provide continuity of care.
