Job summary

Hospice of the Midwest in Woodbury, MN seeks an experienced RN Case Manager to deliver patient-centered hospice care. You will assess needs, develop care plans, administer treatments, and coordinate with a multidisciplinary team to support patients and families.

This role offers Mon-Fri 8:00 AM - 5:00 PM hours, 1:1 patient care, strong documentation, and opportunities for growth within a values-driven hospice organization. Competitive benefits and mileage reimbursement provided.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the state.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or related setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a valid insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
