Resource 1 Homecare Staffing is seeking a Hospice Registered Nurse (RN) Case Manager in Maple Grove, MN. The role involves coordinating hospice care, education, and family support for patients in home and facility settings.

The RN Case Manager will assess patient needs, develop and revise care plans, collaborate with physicians, and guide families through end-of-life care. Expect a Monday–Friday schedule with on-call duties and a competitive compensation package.

Qualifications

• 1-2 years of acute care experience in an institutional setting (required). 1-2 years of home care experience (preferred). We’re excited to train RN's that are passionate about hospice care.

Responsibilities

• Completes initial, comprehensive and ongoing assessments to determine hospice needs.
• Provides nursing care using the nursing process.
• Develops and initiates the plan of care with goals based on nursing diagnosis.
• Coordinates patient care with the interdisciplinary team.
• Educates patient and family on care, medications, and home safety.
• Assists patient with activities of daily living and comfort measures.
• Maintains documentation and communicates changes with physicians.
• Participates in on-call duties as defined by policy.
